Health services and situation

Health services and situation refers to both the country (e.g. disease and epidemics), the healthcare system (e.g. working conditions, quality, cost), and the individual (health status).

Studies listed under this migration driver refer to health services, healthcare provision, healthcare quality and costs, epidemics, health situation, and safety in the health services.

Impact of socio-economical inequalities in the progression of HIV infection at individual and contextual level in Europe

Year 2012
Taxonomy View Taxonomy Associations
Abstract
'Background. The HIV epidemic is a major public health issue in Europe. Over the last decade significant changes in the epidemic have taken place with a increasing proportion of HIV infections being in migrants and women. In turn, these groups might not fully benefit from life-saving antiretroviral treatments (ART), because of barriers to HIV testing and to ART. To date no study with sufficient sample size has explored the role of socio-economic determinants in heath of HIV infected patients in Europe. The role of contextual factors in the health of HIV patients, such as ethnic density, and lower area income level, have not been studied. Objective. We aim to study the socio-economic determinants of inequalities in HIV diagnosis, disease progression, and treatment initiation in Western Europe. Moreover, we will explore the extent to which the variability of HIV prognosis is attributable to either individual (gender, ethnicity, social class) or contextual factors (income area, ethnic density). We will use the routinely collected data of HIV infected patients that will be merged by the newly established EuroCoord collaboration. Methods. The routinely collected database from the newly established EuroCoord collaboration provides information on socio-economic determinants on 250,000 HIV infected individuals from various EU countries. HIV disease progression will be defined as rate of CD4 decline or time to AIDS or death. Marginal structural models to account for time varying confounding will be used to explore differences by socio-economic groups of the effect of ART on the risk of AIDS and mortality. Multilevel and latent variable models will be used to explore the role of contextual factors on HIV progression. Public health implications. This research will help EU health care planners to identify and target groups of HIV infected individuals likely to face barriers to HIV testing and ART and thus prevent HIV-related mortality in more vulnerable populations.'

Primary Health Care of people without papers in Germany - health care access and medical needs

Year 2008
Taxonomy View Taxonomy Associations
Abstract
The proposed study group aims at a systematic investigation of needs for and structures of medical assistance of migrants without legal residence status ("sans papiers") in Germany. No reliable data exist about consulting causes, morbidities, ways of treatment and existing network structures in handling these groups of migrants. In the preliminary studies it is planned to establish contacts with all relevant non-governmental organizations nationwide. First semi-structured interviews will be conducted to investigate which further in-depth methods, like participating observation, ethnography or narrative interviews of migrants are useful, meaningful and possible to use.

Coordinating resources to assess and improve health status of migrants from Latin America

Year 2011
Taxonomy View Taxonomy Associations
Abstract
European health systems are committed to meeting the challenge of understanding the needs of migrant populations and adapting their services to meet these needs. The difficulties inextricably linked to this challenge are caused by the complexity of migration patterns and the differences between migrant population across EU countries. At present, the limited available data show that attempts to incorporate migrants’ health needs, in particular those of migrants from non-EU countries, into EU health systems have remained scattered and uncoordinated. COHEMI’s general objective is to coordinate referral centres dealing with specific Latin American (LA) diseases in order to provide a clear understanding of the full migration cycle in relation with the health systems in Europe and Latin America and to provide a in-depth insight into priority health-related aspects of LA migration in order to facilitate the development of and transfer of evidence and information relevant to migrant health policies.

MOHPROF: Mobility of Health Professionals

Year 2008
Taxonomy View Taxonomy Associations
Abstract
In the medium-scale collaborative project with partners inside and outside the EU, scientific institutes with the capacities to conduct sound investigations will cooperate with worldwide active international health service organisations which have information and global links for research on international mobility. General objective is to research on current trends of mobility of health professionals to, from and within the EU. Research will also be conducted in Non-European sending and receiving countries, but the focus lies on the EU: comparative studies in a selected range of representative states will determine the impact of different types of migration on national health systems. An innovative approach will generate more comparable, specified and qualified data gathered by mainly qualitative research and aims for quantities of migration flows as well as detailed qualities like professions, motives, circumstances and the social context, i.e. push and pull factors. Crucial for the approach are key stakeholders which represent the relevant categories in national health systems to collect existing data and statistics, but first of all to generate new, qualitative data. In-depth interviews based on thematic guidelines with representatives of key stakeholders enable a triangulation of data, i.e. the expertise on health professionals´ mobility and its impact on structures and processes of health systems will qualify the quantitative findings and explore what mobility means for the health system and the persons and organisations involved. The project’s policy dimension comprises recommendations on human resource policies in European and third countries for policy and decision makers on the basis of sound empirical research with conceptual frameworks for monitoring systems concerning the mobility of health workers as a key part. Consultation meetings and roundtables with policymakers will be essential in the project.
